Transition Coordination (Home Again)
The Home Again program is designed to transition people who have been in a skilled nursing facility for at least 90 days and wish to return to the community. Our goal is to optimize the participant’s life choices and rights while minimizing threats to the participant’s safety and health. We also aim to provide a mechanism for managing access to institutional care alternatives.
To be eligible for the Home Again program, an individual must:
- Have been living in a qualified facility for at least 90 days
- Be enrolled in Medicaid for at least a day before the transitioning process
- Meet the required level of institutional care

Expanded Goods and Services
Assistance may include security and utility deposits, furnishing, basic household items, environmental modifications to make a home or apartment accessible, and a wide array of other connections with community resources.
